IGF::OT::IGF FOR OTHER FUNCTIONS JANITORIAL SERVICES EASTER SEALS, EAST GA, INC. AUGUSTA, GA THE PURPOSE OF THIS SUPPLEMENTAL AGREEMENT IS TO MAKE THE FOLLOWING CHANGES BY MUTUAL AGREEMENT OF THE PARTIES: 1. EXCERCISE OPTION II FOR THE PERIOD OF 12/01/2012 - 11/30/2013. 2. INCORPORATE WAGE DETERMINATIONS 2005-2135 REVISION 13 (6/13/2012), IMPARTING A .12 CENT INCREASE FROM $3.59 TO $3.71 PER HOUR FOR HEALTH AND WELFARE. THE JANITORIAL LABOR PRICE REMAINS UNCHANGED. 3. INCORPORATE NISH'S PROPOSAL DATED 10/11/2012. NEW MONTHLY PRICE: $8,933.03 NEW ANNUAL PRICE: $107,196.36 IN CONSIDERATION OF THE MODIFICATION(S) AGREED TO HEREIN AS COMPLETE EQUITABLE ADJUSTMENTS FOR THE CONTRACTORS CLEANING SERVICES TO BE AT THE AUGUSTA, GA FEDERAL BUILDING AND COURTHOUSE DATED 10/11/2012 (PROPOSAL(S) FOR ADJUSTMENT). THE CONTRACTOR HEREBY RELEASES THE GOVERNMENT FROM ANY AND ALL LIABILITY UNDER THIS CONTRACT FOR FURTHER EQUITABLE ADJUSTMENTS ATTRIBUTABLE TO SUCH FACTS OR CIRCUMSTANCES GIVING RISE TO THE PROPOSAL(S) FOR ADJUSTMENT EXCEPT FOR THOSE MANDATED BY THE DEPARTMENT OF LABOR.
