IGF::CT::IGF PATIENT CENTERED COMMUNITY CARE (PCCC) THE REQUIREMENT IS AN EFFORT TO CREATE CENTRALLY SUPPORTED HEALTHCARE CONTRACTS (TO INCLUDE ALL MEDICAL AND SURGICAL SERVICES ALONG WITH SKILLED HOME HEALTH) ON A NATIONAL BASIS IN ALL VETERAN INTEGRATED SERVICE NETWORKS (VISNS). THE GOAL IS TO PROVIDE VETERANS COORDINATED, TIMELY ACCESS TO HIGH QUALITY CARE FROM A COMPREHENSIVE NETWORK OF VA AND NON-VA PROVIDERS TO SUPPLEMENT VA CARE WHERE IT IS NOT AVAILABLE.
