This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. The purpose of this Sources Sought Announcement is for market research only and will be used to help make appropriate acquisition decisions. This is not a solicitation and this notice shall not be construed as a commitment by the Government to issue a solicitation or award a contract. Any information received as a result of this sources sought does not restrict the Government to any type of solicitation, if one is issued. All inquiries are to be made in writing by e-mail to kimberly.burgh@va.gov and any responses to this notice shall be sent by e-mail by 06/15/2018 at 4:00PM CST. Description of Requirements: Government is seeking contractor to provide Community Nursing Home Services based on Medicaid RUG rates as an all-inclusive per diem rate in Houston and surrounding counties listed below: Angelina County Brazoria County Fort Bend County Galveston County Harris County Jefferson County Montgomery County Walker County The Government requests capability statements and comments from interested businesses regarding the requirements described above. Responsible sources are encouraged to submit a response to this notice with a statement of interest on company letterhead. At a minimum, the following information shall be provided: 1. Company Name; 2. Company Mailing Address; 3. Point(s) of Contact including telephone number(s) & email address(es); 4. Socio-Economic (i.e. Small/Large Business, HUBZone, Service Disabled Veteran Owned, 8(a), etc.) as it relates to NAICS Code 623110 Nursing Care Facilities (Skilled Nursing Facilities) ($27.5 Million). 5. Provide a summary of the type of services that your CNH facility provides. 6. Company DUNS number and confirmation that company is registered in System for Award Management, www.sam.gov. 6. Additional information and/or comments. Responses will be shared with government personnel, but otherwise will be held in strict confidence. Contractors submitting comments may also request to discuss their comments directly with the government. It is the intent of the Government to issue a competitive multiple award Indefinite Delivery Indefinite Quantity (IDIQ) type solicitation after all market research has been completed.
