The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Obesity Prevention Case Manger to provide 2080 hours of services at the McLaughlin Indian Health Center in McLaughlin, South Dakota. The period of performance will be Date of Award through September 30, 2011. The Contractor is to perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-11-001, shall be in writing by email. In the written request please reference RFQ number RFQ-11-001 and provide an email address where the RFQ package can be sent. Please also provide the name of the point of contact and the physical address. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be November 10, 2010. The closing date of the RFQ package is November 25,2010. This is a full and open competition requirement.
