Optometrist Services

The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas intends to issue solicitation RFQ-30902-0034-08 for the provision of Optometrist Services to be performed in the Health Services Department(s) for the inmate population at the Federal Correctional Institution/Federal Prison Camp (FCI/FPC) located at 15801 S.W. 137th Avenue, Miami, FL 33177 and the Federal Detention Center (FDC) located at 33 NE 4th Street, Miami, FL 33132. It is estimated that the contract will require four (4) clinical visits per month at the FCI/FPC each of which will normally be six (6) hours in duration during which time an estimated 16 examinations will be performed. It is estimated that the contract will require two (2) clinical visits per month at the FDC each of which will normally be four (4) hours in duration during which time an estimated 15-20 examinations will be performed. Services at the FCI/FPC will normally be scheduled between the hours of 8:00 A.M. and 3:30 P.M., Monday through Friday, excluding Federal holidays. Services at the FDC will normally be scheduled between the hours of 7:30 A.M. and 3:30 P.M., Monday through Friday, excluding Federal holidays. The contract period shall consist of one 12 month base year and 4 twelve month option periods for renewal. The applicable North American Industrial Classification (NAICS) code is 621320, with a small business size standard of $7.0 million. The solicitation is 100% set-aside for small business. All responsible small business sources may submit a quote which will be considered. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The solicitation will be available on or about November 20, 2008 and will be distributed solely through the website at www.fbo.gov. All future information concerning this acquisition, including solicitation amendments will be distributed through the website. The anticipated deadline for receipt of quotes is December 22, 2008.
