Q--CNH Basic Ordering Agreement

Under Statutory Authority 41 USC 253 (c )(5) and 38 U.S.C. 1720 the Government anticipates entering into sole source acquisition negotiations for placement and services of eligible veterans into two nursing home facilities located in Minneapolis, MN as follows: Evangelical Lutheran Good Samaritan Society d/b/a Good Samaritan Society-Ambassador, 8100 Medicine Lake Road, Minneapolis, MN 55427-3404; Evangelical Lutheran Good Samaritan Society d/b/a Arlington Good Samaritan Center, 411 - 7th Avenue NW, Arlington, MN 55307-2156; Evangelical Lutheran Good Samaritan Society d/b/a Comforcare Good Samaritan Center, 205 - 14th Street NW, Austin, MN 55912-4644; Evangelical Lutheran Good Samaritan Society d/b/a St. Croix Valley Good Samaritan Center, 750 East Louisiana, Saint Croix Falls, WI 54024-9501; Evangelical Lutheran Good Samaritan Society d/b/a Good Samaritan Society-Stillwater, 1119 Owens Street North, Stillwater, MN 55082-4316; Evangelical Lutheran Good Samaritan Society d/b/a Good Samaritan Society-University Specialty Center, 22-27th Avenue SE, Minneapolis, MN 55414-3102; Evangelical Lutheran Good Samaritan Society d/b/a Sunwood Care Center, 200 South Dekalb Street, Redwood Falls, MN 56283-1913; Extendicare Homes, Inc. d/b/a Texas Terrace Care Center, 7900 West 28th Street, Minneapolis, MN 55426-3011; Extendicare Homes, Inc. d/b/a Health & Rehabilitation of New Brighton, 825-1st Avenue NW, Saint Paul, MN 55112-6846. This acquisition is for VA Medical Center Minneapolis, MN, VISN 23. The Basic Ordering Agreements will be set up for a base year with 4 one-year options. The above authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by services available from one source in a specific area of responsibility. This action is for services which the Government intends to solicit and negotiate with only one source under FAR 6.302-5 and under commercial item format prepared in accordance with the format in Federal acquisition Regulation (FAR) 13 as supplemented with additional information included in this notice. This notice of intent is not a request for competitive proposals, the reference /solicitation number is VA-263-09-RQ-0074 and issued as a request for quotation (RFQ) notice. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive acquisition.
