Specialized Compounded IV Solutions

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 146 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and issue Blanket Purchase Agreement(s). This is a request for quotation (RFQ). The solicitation number is W81K0218Q0105. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. The Standard Industrial Classification Code is 6515, size standard is 1250 employees and the NAICS code of 325412. This procurement is being solicited as a 100% Small Business Set-Aside for Brand Name or Equal products. The Government intends to award a Blanket Purchase Agreement(s) for a period of 5 years. Contract Requirements - The successful quoter shall be required to provide the Brand Name or Equal requirement for specialized compounded IV solutions from the vendor product lines that meet the salient characteristics given in the DESCRIPTION OF REQUIREMENTS section of attachment; W81K0218Q0105 Attachment A.
