CHADIS data documents guideline-based care, Bright Futures, Patient Centered Medical Home and MOC-4 credit from ABMS. Care Coordination functionality in CHADIS uses online consent forms transmitted by email or fax to send referrals and receive tracking information, supporting a system of care. CHADIS makes statistical control charts for population health monitoring. CHADIS has >2M patients enrolled in 47 states and 10 countries.
