Audiometric Booths for NHCCP, NC

Combine Synopsis/Solicitation # HT940626QE009 is issued as a Request for Quote (RFQ).  The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attachment. The DD Form 1449 is being utilized to generate applicable clauses.

The Defense Health Agency, Healthcare Contracting Division- Southeast intends to procure a firm-fixed price contract for Audiometric Booths in support of the Occupational Medicine Clinic at Naval Health Clinic Cherry Point, NC. The contractor shall provide two (2) Audiometric Booths simultaneously, disassemble, remove and dispose of the two (2) existing audiometric booths, complete installation, and validations within seven (7) business days of start of installation for the new audiometric booths. In addition, the vendor shall include audiometric booth freight/shipping, loading and unloading, all labor, booth certification, and sound field calibration. This requirement is being solicited in accordance with Federal Acquisition Regulation FAR Part 12.6; Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services.

Competition Approach: Full and Open Competition.

North American Industrial Classification System (NAICS): The associated NAICS code for this procurement is 512240 with a small business size standard of $11.0M.

Submission of Quotes: Offerors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Quote Instructions and Evaluation Criterion provided in the solicitation document to create the quote. Quote is required to be submitted electronically to the Contracting Officer and Contract Specialist on or before the date and time specified in the RFQ. The firewall utilized by the Government contracting office does not authorize or accept zip files. Files shall not contain classified data. The use of hyperlinks in quotes is prohibited.
