Optometry Services for Blue Grass Army Arsenal

This is a sources sought notice of the Government's intent to assess interested parties for the procurement of Optometry Testing Services within 50 miles of Bluegrass Army Depot in care of Ireland Army Health Clinic, Fort Knox, KY. The Government estimates that there will be 20-30 tests done during the first year but there are no guaranteed minimums or maximums associated with this request. The testing that will be required is as follows: New Patient Office Visit / Level B Screening EYE EXAM RX ONLY Scuba inserts (SCBA) Parties interested in future announcements on this procurement shall provide detailed information of their capabilities and certifications to clearly meet the requirements stated above. Interested parties must be registered in the System for Award Management (SAM), https://www.sam.gov/portal/public/SAM/. Responses(s) may be via email or on company letterhead: include company's name, point of contact, address, phone number, DUNS number, Tax ID, business size.
